After reviewing the policies and procedures, please complete the IFES parent/student signature form at the link below. Please submit a new form for each child at IFES. All forms are due by Friday, September 8, 2017.

SMSD Acceptable Use Policy Agreement
READ & DISCUSS - This is a single page version of the AUP. While it no longer needs to be signed annually, the purpose of the agreement is to confirm the student's intent to comply with the district policy regarding appropriate access and utilization of district devices, network, and online resources. READ - This document is provided for you to read the terms and conditions of the South Middleton School District's B.Y.O.T. (Bring Your Own Technology) and Google Enterprise (Google Applications for Education) programs. Students at the Boiling Springs High School are automatically a part of these programs, and teachers are encouraged to use as much technology tools and resources with our students as often as possible.

**Print & Sign** - Use this document to give your child permission to fully participate in the district's personalized blended learning program. NOTE: If this student participated in the program in the past and already has a Google account, there is no need to submit a new form.
